ANGIOGRAPHIC EVALUATION OF TRANSPLANT KIDNEY

血管攝影檢查對移植腎臟的評估

摘要

Angiographic studies were performed in 14 renal transplant recipients for evaluation of the possible complications. In 8 biopsy proved cases, hyperacute rejection was diagnosed in 1 case, acute tubular necrosis in 2 cases, and chronic rejection in 5 cases. In the other 6 cases, normal angiography were seen in two cases, rejection phenomenon were suspected in three cases, and ureteral obstruction was confirmed surgically in one case. We suggested that angiography is good adjunct to ultrasonography for the diagnosis of post-transplant dysfunction, particularly for renal vascular problems.

自民國74年3月到民國76年2月，本院共有14例接受過腎臟移植的病人實行了血管攝影檢查，以評估手術後可能發生的併發症。在8例接受移植腎臟活組織檢法之受腎者中，有6例可以看到排斥現象的血管攝影表徵，其中包括1例極急性排斥與5例慢性排斥。其餘兩例為急性腎小管壞死者，血管攝影所見別是正常。在未經組織病理診斷之6例中，有兩例之血管攝影所見是正常的，有三例疑為排斥現象，另一例經外科手術證實為輸尿管阻塞。其中疑為急性排斥的病人中有一例併發有明顯的腎動脈狹窄。血管攝影檢查在腎臟移植後功能障礙之原因的判斷上是個準確可信的方法，尤其是在血管方面的問題。
