以複合杜卜勒超音波診斷自發性腎臟之動靜脈瘻管

Spontaneous Renal Arteriovenous Fistula-Diagnosis by Duplex Ultrasound

摘要


腎臟的異常動靜脈交通可以分為先天性動靜脈形成異常,創傷性動靜脈瘻管,及自發性動靜脈瘻管(arteriovenous fistula, AVF)。除了因腎腫瘤侵入腎靜脈而造成自發性瘻管外,最常見的自發性AVF多為先天性或不明原因者。診斷腎臟自發性AVF主賴腎血管攝影術。據我們所知,以往並無報告提及以複合杜卜勒超音波診斷出腎臟的自發性AVF,故我們在此報告一例此類病人,其初步腎臟複合杜卜勒超音波檢查即已診斷出腎竇部的囊狀結構屬於動脈性結構,而低血流阻力係數指出有AVF的可能,其後並由血管攝影術証實為AVF無誤。

Abnormal arteriovenous communication of the kidney can be classified as congenital arteriovenous malformation, traumatic arteriovenous fistula, and spontaneous arteriovenous fistula(AVF).Spontaneous AVF is most commonly congenital or idiopathic except those caused by venous invasion in renal tumor. The diagnosis of spontaneous AVF is mainly based on renal angiography. To our understanding, there has been no previous report of diagnosing a renal AVF using duplex ultrasound. We herewith decribe a case of spontaneous renal AVF. An initial duplex ultrasound study for evaluation of cause of hematruia revealed an unusual low-resistance vascular structure which is highly suggestive of AVF, and the correct diagnosis is thereafter confirmed by renal angiography.
