經動脈門靜脈肝臟電腦斷層評估肝腫瘤:比較超音波、電腦斷層及血管攝影

Arterial Portography with Computed Tomography to Evaluate Hepatic Tumor: In Comparison with Ultrasound, Computed Tomography and Angiography

評估35位肝臟影像執行超音波、電腦斷層(computed tomography簡稱CT)、血管攝影及經動脈門靜脈肝臟電腦斷層(arterial portography with computed tomography簡稱CTAP)影像,臨床診斷為25位肝癌、3位結腸癌、3位肝血管瘤及1位充血性心衰竭而無任何肝腫瘤,其中有兩位病患超音波誤診為有肝腫瘤,其餘33位病患中CT僅1位比超音波多發現腫瘤,而血管攝影有6位沒看到腫瘤,但有7位比超音波多發現腫瘤數目,2位有動門靜脈交流現象;而CTAP可高達13位(39.4%)病患比超音波多診斷出腫瘤數目,7位有扇形無顯影區,有兩位做血管攝影可看到動門靜脈交流。綜合超音波、CT、血管攝影三項檢查,CTAP尚有8位病患多診斷出腫瘤數目,CTAP是一項高敏感率的影像檢查,它不僅能檢查出更多腫瘤數目,且具有更正確而清晰的解剖位置,可做為肝腫瘤病患開刀前之影像評估。

Imaging of the liver was carried out with ultrasonography, computed tomography, angiography and arterial portography with computed tomography(CTAP) in 35 patients with suspected hepatic tumors. Subjects included in this study comprised of 28 patients with HCC, 3 with colon cancer, 3 with hemangioma, and one with congestive heart failure without a hepatic lesion. Two patients were misdiagnosed sonographically. In the remaining 33 patients, CT detected mass lesions in additional to that noted sonographically in only one patient. Angiography was able to demonstrate additional tumors in seven more patients. However, ultrasonography detected more tumors than angiography in 6 patients. CTAP revealed otherwise undetected tumors in addition to those found sonographically in 13 patients(39.4%), 7 of whom demonstrated wedge-shaped nonenhancing areas in the liver. Arterio-portal shunting was comfirmed angiographically in 2 of these patients. With the combined detection rates of ultrasound, computed tomography and angiography, additional tumors were still disclosed after CTAP. CTAP is an extremely sensitive imaging modality capable of a higher tumor detection rate with clearer anatomic delineation, and may therefore be employed in the routine evaluation of the patients with hepatic tumor.
