以磁振造影磁化向量空間調變技術研究局部心肌運動

A New Method to Assess Regional Heart Wall Motion by MRI Using Spatial Modulation of Magnetization

摘要


本文之目的在建立一套安全。準確的檢查方法,來研究人體局部心肌運動。方法上以在PICKER 1.0T HP磁振造影機,自行設計磁化向量空間調變(spatial modulation of magnetization)的標定技術,取得有網狀條紋標示的心臟磁振造影像,希望藉由此非侵襲性的方法,觀察左心室肌肉層內小正方形格子,在心搏運動週期中,形狀改變之情形,可以更直接、更準確的評估左心室局部心肌運動,並對於探討心肌運動模式和心肌動力學有更多之助益。同時也將國內所實施之傳統解剖學、形態學之磁振造影檢查推展到功能性、生理性之臨床運用。

Previously, the direct assessment of regional heart wall motion required invasive methods such as the implantation of pulse transit sonomicrometry crystal or fluoruscopically trackable metallic marks within the myocardium. Their applicability in human studies was therefore limited, even the 2-D cardioechography applied since the end of 1970's had limitation in identifying endocardial and epicardial boundaries and measuring cavity dimension and wall thickening. MRI has been used since recent years to assess the regional heart wall motion because of its possible potential in the study of cardiovascular disease. Being great potential in the study of cardiovascular disease, MRI has been used to assess the regional heart wall motion. We tried to use the tagging technique of SPAMM (spatial modulation of magnetization), created by Dr. Leon Axel in 1989, to produce images with a regular pattern of stripes that moves with the heart wall and to permit the use of a 2-D grid of stripes for more detailed studies of heart wall motion. It provides a unique method of analyzing regional heart wall strain.
