Therapeutic Arterial Embolization of Spontaneous Hemorrhage from Ruptured Renal Angiomyolipoma

經腎動脈血管栓塞治療自發性破裂併出血之腎臟肌肉血管脂肪瘤

摘要


這是一例以選擇性經腎動脈血管栓塞來處理自發性破裂腎臟肌肉血管脂肪瘤的例子。患者因為左腰突然劇痛,至本院急診求診。經腎臟超音波掃描及腹部電腦斷層攝影判讀左側腎臟肌肉血管脂肪瘤,合併腎臟周圍血腫。以選擇性經腎動脈血管攝影證實此一腎臟肌肉血管脂肪瘤典型特徵。隨後以止血芳(gelform)成功地完成高度選擇性腎動脈血管栓塞。患者於術後四天出院,並無併發症。三個月後之腹部電腦斷層攝影追蹤顯示整個腫瘤無血腫,同時患者並無任何症狀。因此我們認為除了外科手術以外,經皮腎動脈血管栓塞,對於處理自發性破裂腎臟肌肉血管脂肪瘤不失為-安全又有效之另一選擇。

A case of spontaneous rupture of renal angiomyolipoma was managed by selective therapeutic arterial embolization. The patient was admitted via our emergency room due to sudden onset of severe left flank pain. Renal ultrasonography and abdominal computed tomography (CT) scan findings were suggestive of left renal angiomyolipoma with perirenal hematoma. Selective renal angiogram confirmed the classic features of angiomyolipoma and selective arterial embolization using gelfoam cubes was successfully carried out thereafter. No serious procedural related complications were noted and she was discharged four days after the embolization. Follow-up CT scan of the abdomen three months later revealed no more hematoma and she was asymptomatic. In conclusion, therapeutic arterial embolization is a safe and effective alternative to surgical intervention in the management for rupture of renal angiomyolipoma.
