Conservative Surgery with Radiotherapy for Early Stage Breast Carcinoma-Preliminary Review

早期乳癌之乳房保留術及放射治療的初部評估

摘要

自1994年12月至1996年12月共有10位女性早期乳癌病人接受乳房保留治療術及放射治療。年齡分佈爲28至35歲。其中9例是T2N0M0，1例是T2N1M0，6例接受局部腫瘤切除及同側腋下淋巴清除，4例接受乳房四分之一切除及腋下淋巴清除。10位病人術後接受放射治療，照射範圍只照射乳房，總劑量是5,000 cGy分成28次照射，使用的機器是6MV直線加速器光子射線。其中1例因淋巴轉移在放射治療後接受6次的Cyclophosphamide, Methotrexate及5-Fluorouracil的化學治療，平均12個月的追蹤時間裡，10位病人目前是沒有復發及轉移現像。10例病人對於治療效果滿意並無乳房纖維化及縮小現像。

A preliminary review was conducted for ten female early breast cancer patients treated with conservative surgery and radiotherapy from December 1994 to December 1996. Patients' ages ranged from 28 to 53 years old. Nine patients were staged clinically as T2N0M0 with one patient staged as T2N1M0. Six patients were operated with lumpectomy and axillary lymph node dissection, while 4 patients were treated with quadrantectomy and axillary lymph node dissection. All 10 patients were treated with radiotherapy to the chest wall by tangential method using 6 MV linear accelerator photon beams. Total dose of 5,000 cGy was given in 28 fractions. The patient with lymph node metastasis was treated adjuvantly with 6 courses of chemotherapy using Cyclophosphamide, Methotrexate and 5-Fluorouracil. With a minimum follow up of 12 months, all patients are alive with-out evidence of diseases. All patients are also satisfied with the cosmetic results of the treatment with no significant fibrosis or shrinkage of the treated breast noted.
