條碼在醫學影像儲傳系統的應用

The application of Bar-Code in Picture Archiving and Communication System

摘要


條碼的應用已成為我們日常生活的一部份,為了研究條碼對醫學影像儲傳系統(Picture Archiving and Communication System, PACS)的實際效能,我們利用條碼讀取機作為放射科收件、檢查、及製作報告的輔助工具,我們先完成健診、重症加護病房及急診使用條碼搭配著PACS作業。而今數位影像X光檢查較傳統X光檢查的作業流程複雜,包括從放射科電腦收件、列印收件單、然後在工作排程清單(work-list)選擇檢查項目、設定檢查參數、攝影檢查、影像讀取、影像品管、傳送到PACS主機、影像判讀及申請出片等。我們研究目的是希望利用條碼的讀取,減輕現場放射師的工作負荷,減少鍵盤輸入的錯誤;讓醫師快速擷取醫院資訊系統(Hospital Information System, HIS)及放射科資訊系統的資料(Radiology Information System, RIS)的資料,直接從影像顯示器(viewer)判讀X光影像及製作報告。本研究對隨機抽樣102人次之觀察結果顯示,在未使用條碼時一般檢查資料輸入約平均花費7秒,CT、MRI為47秒,錯誤率為2.95%。條碼系統的應用後,我們對56位病患以條碼讀取方式擷取病患醫囑資料作比較,每筆基本資料的輸入擷取只需1秒,錯誤率為0%。此外,放射師執行X光檢查時,透過本院自行開發的「X光收件/補收件」軟體,準確地從HIS/RIS擷取病人資料,不須鍵盤輸入及設定檢查參數。放射科醫師直接透過「X光報告」製作軟體,利用條碼快速同時讀取HIS/RIS資料及影像優點,提升製作報告的效能, 避免人為輸入的錯誤。總言之,在本研究中並未發現有條碼讀取錯誤的情形,因此利用條碼可強化放射師及醫師在PACS的實際工作效能。

Bar-code system is widely utilized in our daily life. In order to assess the usefulness of the bar-code system in picture archiving and communication system (PACS), we used bar-code reader as the complementary tool of PACS for registration, radiographic taking, and imaging reporting in the department of the diagnostic radiology. Our hospital has just accomplished PACS with bar-code system for the Health Care Center, Intensive Care Unit and Emergency Department. The working flow of our computer radiography (CR) is currently more complicate than that of the conventional radiography. It requires registry, printing registry sheet, selecting the items of the procedure from the work-list and seting imaging parameter in the CR ID terminal, taking radiography, reading imaging by CR reader , imaging quality assurance, transporting images to the PACS, printing images, and film interpretation, In this study, we used bar code in PACS for the purposes to reduce the workload and the keyboard input error of technologists, and to facilitate radiologists to interpret the images on PACS viewers by rapid retrieval of Hospital Information System (HIS) and the Radiology Information System(RIS). From an analysis of 102 randomized cases using key-broad to input examination information, we found that the averaged time for the routine radiologic procedures was 7 seconds, while for CT and MRI was 47 seconds. The error percentage is 2.95%. For comparison, we assessed the efficiency of bar code in retrieving medical information from 56 patients. We found that each data retrieval with bar code required only one second and the error percentage was zero. In addition, radiologists took the advantage of bar code by getting the images, clinical and radiological data simultaneously, obviating and input mistake. We concluded that bar code could enhance the efficiency of technologist and radiologist during executing PACS.
