As the rapid growth of the elderly population and chronic illnesses, the Telecare Pilot Project has been conducted to address the growing needs in long-term care in Taiwan. The Project is commissioned by Department of Health, Executive Yuan, and executed by ICL/ITRI. A multi-disciplinary team has been formed including members from medicine, health care, medical devices, information & communication technologies, and security. The team’s objective is to develop a comprehensive telecare service model that covers the community-based care, home-based care, and institutional care services. The integration of the health services, care information, and related resources is supported by the telecare information integration platform. This comprehensive Telecare service model and information delivery infrastructure enable continuous long-term care services.
