懷孕中的轉移性卵巢癌
Pregnancy Complicated with Metastatic Ovarian Cancer
摘要
懷孕中的卵巢腫瘤，對於懷孕婦女本身或是她的產科醫師而言，均是一個極大的挑戰。主要的原因是必須同時兼顧懷孕的婦女本身以及腹中的胎兒，更何況懷孕中也常常伴隨一些生理性的囊腫，在懷孕初期扮演助孕的作用，一但懷孕終止或結束以及隨著妊辰期的增加，絕大部分均會消失。我們在此報告一個32歲的婦女，在3年前因為大腸癌接受下段直腸的切除及一連串的化療後狀況穩定，於是於三年後自然懷孕。在懷孕時意外現兩側卵巢腫瘤，於是在第十七週時接受剖腹探查，發現是大腸癌的復發轉移到卵巢，於是在本院接受第2次手術的切除，術後並無化療。在三十八週選擇剖腹產，生下健康的男娃，腹腔內並無殘存腫瘤存在。目前仍在追蹤中，已兩年，母子均安。本文就藉由這位婦女的病程，來介紹此病。

Ovarian tumors, especially malignant type, might complicate pregnancy in any one of the pregnancy periods, and any management used should not have a negative impact on the wellbeing of either the mother or the fetus. Herein, we present a pregnant woman, who had a history of colon cancer 3 years ago, with diagnosis of metastatic ovarian cancer at 17 weeks of gestational age. She was treated with bilateral salpingo-oophorectomy with successful delivery of a male baby at 38 weeks of gestational age through Cesarean section. Now she has had a disease-free survival of two years and the baby has grown without delayed development.
