Toxoplasmosis is a worldwide zoonotic protozoa infestation with high prevalence. Up to 15% of unexplained cervical lymphadenitis had been reported. A 37 -year-old female presented to our clinic with an unexplained right posterior neck mass. Excisional biopsy showed characteristic histologic feature of toxoplasmic lymphadenitis which was later confirmed by serological test (enzyme-immuno-assay IgG antibody test). Prophylactic oral antibiotic was given due to the propagation of lymphadenitis to the inguinal area. The patient was well after 6 months of follow-up. We reviewed the literature and discussed the case. Although toxoplasmic lymphadenitis is self-limited in immunocompetent people, toxoplasmosis in immunocompromised patients and in pregnant women may result in mortality. Toxoplasmic lymphadenitis should be included in the differential diagnosis of cervical lymphadenopathy.
