Frontal Sinus Inflammatory Pseudotumor Caused by Sinus Barotrauma

摘要

發炎性偽腫瘤（Inflammatory pseudotumor）是鼻竇很少見的一種疾病。我們報告一個案例，是搭乘民航機時，因鼻竇氣壓傷害所造成的額竇發炎性偽腫瘤。患者在搭乘飛機時，感受到左側額竇劇烈疼痛，臆斷是鼻竇氣壓傷害，其臨床分級為Weissman's class III。鼻竇電腦斷層顯示在左側額竇有一同質性低密度（homogenous hypodense）的腫塊。患者接受了鼻竇內視鏡手術，發現在左側額竇裡有一深紅色的腫瘤，該腫瘤在術中已仔細清除，且保留其周圍正常黏膜組織。病理組織學診斷顯示該腫瘤為發炎性偽腫瘤。術後給予病患口服類固醇並逐漸減量，另外加上鼻去充血劑。術後症狀已獲得改善，且追蹤過程中均未有復發情形。（台耳醫誌 2011；46：346－350）

Inflammatory pseudotumor (IPT) is a rare lesion in the paranasal sinuses. We report a case of IPT caused by severe sinus barotrauma during a commercial flight. The patient suffered from severe pain over the left frontal sinus during air travel. Sinus barotraumas was impressed, and the clinical grade was determined to be Weissman's class III. Computed tomography of the paranasal sinuses revealed a homogenous hypodense mass in the left frontal sinus. The patient underwent endoscopic sinus surgery, and a dark-red tumor was found in the left frontal sinus. The tumor was carefully removed without damage to the normal mucosa. Histopathological examination revealed IPT. A tapering course of oral steroids and nasal decongestants was administered. Symptoms were relieved after surgery, and no recurrence was observed.
