肌萎縮性側索硬化症在耳部的表現

目的：本研究的目的在於了解肌萎縮性側索硬化症在耳部之表現。方法：自2011年07月至2012年05月止，17名住於本院祈翔病房之肌萎縮性側索硬化症（ALS）病人納入本研究。17名病人中，14名男性，3名女性。年齡介於50歲及91歲間，平均67.6歲。本研究使用鼓室圖檢測儀做檢測，17名在ALS專責病房之病人左右耳均給予檢查。此外，一併收集每名病人ALS有關之耳鼻喉部份的表現資料。結果：17名病人發病初始症狀，其中10名是以上或下肢麻痺表現，這10名病人之後陸續出現吞嚥困難併或發音困難；另7名病人則是以侵犯延髓為初始表現：2名口語不清、2名臉部麻痺、1名吞嚥困難、2名呼吸困難，這7名病人至今未有上或下肢麻痺現象。17名ALS病人中，12名曾因聽力異常、耳部不適而會診耳鼻喉科，經由耳鏡觀察懷疑有中耳積水共8名，其中有7名ALS病人在之後的鼓室圖檢查為B型。結論：耳部份的症狀也是肌萎縮性側索硬化症常見的表現，中耳功能異常亦非少見。

OBJECTIVES: The study is to investigate otoologic presentations of amyotrophic lateral sclerosis.METHODS: Seventeen patients with amyotrophic lateral sclerosis (ALS) in the Chi-Hsiang ward (ALS dedicated care ward) in Taipei City Hospital were investigated in our study, among them fourteen were male and three were female. The mean age of them was 67.6 years. These patients with ALS had received otoscopic examination, tympanometry on their bilateral ears. The otolaryngologic presentations of each patient with ALS were also analyzed within our study.RESULTS: ten patients out of 17 with ALS showed upper or lower extremities paralysed presentations initially. Dysphagia and/or dysarthria were following occurred out of these ten patient with extremities paralysis. There were bulbar invasive symptoms initially in the other seven patient with ALS. The symptoms of them were slurring speech on 2 patients; facial palsy on 2 patients; dyspnea on 2 patients and dyspnea on the other one patient. Twelve of seventeen patients had consulted otolaryngologist resulting from abnormal hearing or otologic discomfort. Seven of eight patients impressed with otitis media effusion via otoscopic examination were Type B in tympanometry.CONCLUSIONS: Otologic symptoms are common examed in patients with ALS. Abnormal findings in middle ear function can be seen often.
