Unilateral Lingua Villosa Nigra

Black hairy tongue (lingua villosa nigra) is a benign disorder caused by abnormal elongation of the filiform papillae of the tongue, which allows porphyrin-producing chromogenic bacteria or yeast to overgrow and contribute to the discoloration. The location was just anterior to circumvallate papillae and the sides or tip of the tongue was not involved. The color can range from yellowish, brownish to black. Although black hairy tongue is usually asymptomatic, halitosis, metallic taste, nausea, dysgeusia, and tickling sensation may be noted. Unilateral black hairy tongue is extremely rare. Three reported cases were either the patient underwent ipsilateral dental procedures or had ipsilateral trigeminal neuralgia preceding the discover of the unilateral discolored tongue. It is postulated that these patients avoiding rubbing their tongue against the painful area may lead to the unilateral black hairy tongue. In our case, he had the habit of chewing food only by using left side teeth. Therefore, we supposed that this was the reason leading to the unilateral lingua villosa nigra.
