雙側扁桃腺周膿瘍－病例報告
Bilateral Peritonsillar Abscess-Case Report
摘要
雙側扁桃腺周圍膿瘍很少見，發生率目前並不清楚，由於可能阻塞上呼吸道或是擴散至周圍頸部的大血管，因此有潛在性嚴重風險。典型的單側扁桃腺周圍膿瘍的不對稱臨床表現不一定會顯現出來而增加了臨床診斷的困難度。本文報告1例雙側扁桃腺周圍膿瘍，55歲男性，有糖尿病與抽菸病史，因喉嚨痛吞嚥困難而至急診求診，電腦斷層攝影檢查發現雙側扁桃腺周圍膿瘍，經過抗生素治療、切開引流、針筒抽吸等手術，病患恢復情況良好，於門診追蹤至今無復發跡象。本文除了分享我們的治療經驗，並回顧了雙側扁桃腺周圍膿瘍的文獻。

Bilateral peritonsillar abscess (PTA) is a rare condition, however the incidence of bilateral PTA is still unknown. The possibility of compromising the upper airway or spreading to the surrounding structures (including carotid sheath) made bilateral PTA a potentially lethal condition. Clinical diagnosis of bilateral PTA may be difficult because the classic asymmetric findings maybe absent. We report a case of bilateral PTA in a 55 years old man with diabetes medical history and cigarette smoking behavior; patient was send to our ER due to sore throat and dysphagia. Computed tomography (CT) revealed bilateral PTA. He recovered well after antibiotic therapy and surgery (needle aspiration + incision and drainage). No recurrence has been observed until now. We discuss the present case and review the literature of bilateral PTA.
