Parapharyngeal Pleomorphic Adenoma Presenting as Facial Palsy-Case Report

以顏面神經麻痺為首先表現之咽旁間隙多型性腺瘤－病例報告

摘要

The parapharyngeal space is shaped like an inverted pyramid. In this space, benign tumors account for approximately 80% of all tumors that pleomorphic adenoma is the most frequently encountered. It can develop de novo or may arise from the deep lobe of the parotid with extension through the stylomandibular tunnel into the parapharyngeal space. Early symptoms are non-specific, usually not significant. We presented a case of right primary parapharyngeal pleomorphic adenoma having right facial palsy as the initial symptom. Tumor excision was done through the transcervical-submandibular approach. There was a dramatic recovery from facial palsy in the postoperative recovery room. In this article, we described the detailed course of this case.

咽旁間隙為一倒錐狀之頸部空間，其內良性腫瘤以多型性腺瘤為主，約佔80%。咽旁間隙之多型性腺瘤可以是原發性亦可源自於同側腮腺深葉處。大多數病人是沒有臨床症狀或症狀輕微。此篇報告為1名40歲女性，因顏面神經麻痺求診，經藥物治療後完全無改善，因意外發現咽旁間隙腫瘤並於顏面神經麻痺後1個月手術，術後顏面神經麻痺情況立即改善，3個月後追蹤核磁共振無腫瘤殘留或復發且顏面神經功能幾乎回復至正常。臨床上很難從顏面神經麻痺中發現咽旁間隙良性腫瘤，故提出此罕見個案。
