摘要

功能性內視鏡鼻竇手術之嚴重併發症有大量出血、眼窩併發症、腦部或顱底併發症。大量出血併發症大多是因為傷害到外頸動脈的蝶顎動脈分支、內頸動脈、或內頸動脈的前篩動脈分支或後篩動脈分支所造成；眼窩併發症中，後眼窩血腫、視神經受損、眼球肌肉受損為嚴重眼窩併發症。本文將介紹功能性內視鏡鼻竇手術嚴重併發症及其處理方式。充分且完整的解剖與手術知識、詳細手術前評估、及謹慎的手術操作是避免手術併發症最好的方法。

Serious complications of functional endoscopic sinus surgery include massive bleeding, orbital, as well as brain or skull base complications. Massive bleeding is usually associated with violation of the sphenopalatine artery from the external carotid artery, internal carotid artery, anterior or posterior ethmoid arteries from the internal carotid artery. Serious orbital complications included retro-orbital hematoma, optic nerve injury, and intra-orbital muscles injuries. In this study, we report the serious complications of endoscopic sinus surgery and discuss their management. Accurate knowledge of surgical anatomy and proficiency in surgical skills, detailed preoperative evaluation, as well as surgical expertise may minimize the risks of surgical complications.
