Successful preservation of an eye ball and restoration of a useful vision were gained in a case, male, aged 23 years, suffering from a severe penetrating injury. The cornea was cut from upper to lower limbus, the viteous escaped from the corneal wound, showing throuth the closed eyelids. The iris was separated and prolapsed, and the lens was dislocated into the anterior vitreous cavity. The escaped vitreous in the anterior chamber, particularly in the chamber angle was carefully removed. The iris was replaced. The lens substance was removed regardless of the further loss of the vitreous. Finally the corneal wound was closed water tight. The eye recovered rapidly with little inflammatory reaction. Three months after the injury the eye ball remained quiet and the vision was restored to 0.2 corrected with a contact lens. A rising new concept on the management of penetrating injury of the eye was discussed.
