Diagnosis of Glaucoma Current Methods of Examination

Glaucoma is one of the major cause of blindness in Taiwan. More than seven hundred years ago, it was briefly described in old Chinese medical books, and as in ancient time of Hippocrates, was mixed up with some other eye diseases. Today, definition of glaucoma can be made more precisely because of application of new techniques, long term close observation and enlightened knowledge of the disease. Most of the glaucoma are established in the following steps, namely; initial stage of aqueous circulatory disturbance, secondary stage of ocular hypertension, and finally, stage of anatomical change and visual function disturbance. These three steps gradually progress step by step in chronic simple glaucoma, while rapidly develop in acute glaucoma. The diagnosis of glaucoma, therefore, can be proceeded from detection of aqueous circulatory disturbance by examination of outflow facility with electrotongraphy. Furthermore, there are numerous tonometers available for measurement of ocular pressure. Among them, Goldmann applanation tonomter satisfied our present clinical requirement. Visual function can be studied by visual field examination. Goldmann perimeter proved to be the most suitable instrument. Optic disc cupping is another important finding in the study of optic nerve injury. Bilateral camparison for the symmetricity afford valuable information sometimes. Gonioscopic examination is the clue for the classification, diagnosis as well as for operative procedure. Evaluation of these examinations was discussed.
