本報告係收集45例病患之50隻眼。所從事的手術包括白內障、青光眼、視網膜剝離以及眼球內異物摘除手術。無併用enpinephrine,hyaluronidase及xylocaine,只用0.75% Bupivacaine hydrochloride做眼球後麻醉及瞼麻醉。使用總劑量約8至10c.c.。在大部份的眼例中,本劑可維持2至3小時的持續時間。感覺麻醉的發生皆先於運動麻醉;所有感覺麻醉皆在注射後20分鐘之內產生。運動麻醉的發生則顯示着相當大的差異性,其中36眼(72%)在注射之後20分鐘才產生,此和感覺麻醉的發生有顯著不同。本劑的明顯缺點是注射時有疼痛感,而該疼痛感在短暫時間內大都消失。
A study was made of bupivacaine hydrochloride in 50 eyes of 45 cases undergoing cataract, glaucoma, retinal detachment and intra-ocular foreign body surgery. A concentration of 0.75% was used plainly without any combination of epinephrine, hyaluronidase and xylocaine. The total dosage employed was 8 to 10cc. It provided ocular anesthesia and akinesia of 2 to 3 hours' duration among the great majority of cases. The onset of anesthesia was earlier than akinesia in all cases. The onset time of anesthesia was less than 20 minutes in all cases. In 16% it was less than 8 minutes and was longer than 8 minutes among the remaining 84%. The onset time of akinesia showed a wide range of time distribution. In 36 eyes, 72%, however, it was longer than 20 minutes. This contrasted to the earlier onset of anesthesia. The major disadvantage of bupivacaine in ophthalmic surgery is pain on injection, which usually subsided sooner or later.
