人工水晶體植入手術中使用Healon之經驗

The Use of Healon in Introcular Lens Implantation

摘要


Healon是一種含玻尿酸鈉之玻璃體替代物。著者對80例施行人工水晶體植入手術之病例,40例於水晶體植入時以Healon充塞前房,另40例以空氣充塞,每一組40例中各有20例施行囊內白內障摘出術並植入Leiske前房型人工水晶體,及20例施行囊外摘出術並植入Sinskey後房型水晶體。結果顯示,使用Healon之一組在手術過程中困難度較低而全部順利植入,使用空氣之一組於植入時前房蹋陷一次以上者有20例,其中使用前房型人工水晶體者有3例無法繼續推入鏡片而必須放棄植入或改用Healon。Healon組術後角膜厚度增幅較小,視力恢復較快,除了短暫性眼壓上升外無任何嚴重之併發症。Healon之使用使此種手術步驟變為容易和安全。

This paper presents the results of clinical investigation on the effectiveness of Healon (sodium hyaluronate) in the implantation of intraocular lens. In 80 consecutive cases for intraocular lens implantation, air and Healon were used respectively in 40 cases to keep the anterior chamber from collapsing during the insertion of lenses. Leiske anterior chamber lens and Sinskey posterior chamber lens were used on 20 cases each for both air and Healon using groups. The result showed that in the Healon group, there was no noticeable difficulty during the operation and lenses were all successfully implanted. Whereas in the air group, there were 14 cases in which the anterior chamber collapsed more than once during the lens insertion. Of these 20 cases, 3 to be implanted with Leiske lens had to be given up or resorting to Healon because of being unable to keep anterior chamber at all. The corneal thickness increment during the first week postoperatively was lesser in the Healon group than the air group. Except for transient intraocular pressure elevation, there was no any side effect associated with the usage of Healon. It is concluded that Healon makes the implantation surgery much easier and safer.
