白內障術後之內皮細胞喪失與角膜厚度

摘要

我們利用只單側進行過白內障手術之病患131人，拍攝他們兩眼之角膜內皮細胞，以了解手術對角膜的影響，單純的白內障摘除，囊外或囊內引起之內皮細胞喪失是差不多的。2年前之前房人工水晶體置入病例，平均細胞喪失為23.3%，而最近5個月之病例只14.4%，這降低的細胞喪失百分比是手術技巧進步之功效。
我們後房水晶體置入之平均細胞喪失(22.6%)之稍為偏高，是因為我們在這方面之技巧不够熟練，但相信只要假以時日，困難是會克服的。在角膜厚度方面的探討是，單純的白內障摘除，經4個月時間角膜已恢復正常，但若置入人工水晶體的話，則需要較長的時間了。

(1) Bilateral corneal endothelial photographs were taken in 131 cases who underwent unilateral cataract extraction with or without implantation.
(2) No difference of endothelial cell loss was found between simple extracapsular (12.1%) and intracapsular (13.0%) cataract extraction in our cases.
(3) The endothelial cell loss of intracapsular implant in the past 2 years is 23.3% but the recent 5 months is only 14.4%. The decrease of cell loss in recent cases is due to the improvement of our surgical technique.
(4) In the IOL cases, the mean cell loss is relatively high when the IOL was implanted following ECCE (22.6%). The reason is that our technique in the beginning period of PCL implantation is not so skilful.
(5) Central corneal thickness measurement in our cases showed that CCT was normalized 4 months after simple implanted eyes may not be stabilized at the same period.
