頑固性非典型結核病菌引起之角膜潰瘍
Intractable Atypical Mycobacterium Corneal Ulcer
摘要
本文報告五例角膜潰瘍，經病理組織切片及Lowenstein-Jensen培養基證實其病菌為Mycobacterium fortuitum，屬於Runyon氏分類第四群。五例病患均有角膜異物傷害或翼狀贅片手術的過去史。臨床上酷似真菌性角膜炎，病情進行緩慢但具頑固性；對一般廣效抗生素（包括Amikin，Kanamycin）及抗結核病藥物均有高度的抗藥性。5例中4例最後經由全層角膜移值或角膜切除的處理而控制病情；1例卻導至角膜穿孔而發生眼內炎。

This paper describes five cases of intractable Atypical mycobacterial keratitis, 4 male and 1 female. The microorganism was proved, by pathological section or Lowentsin-Jensen medium, to be M. fortuitum which belongs to the Runyon Group Ⅳ, rapid growers. All the patients had a history of corneal foreign body removal or pterygium surgery. The clinical picture was very similar to fungal infection. These corneal ulcers were extremely difficult to treat. At present, there are no satisfactory therapeutic agents or broad spectrum antibiotics (Cefamandole, Kanamycin, Amikin, etc.). The infection led to endophthalmitis in one case. Keratectomy or Penetrating Keratoplasty were performed in the other four cases.
