自體半層角膜旋轉移植術-病例報告

摘要


本文報告二例翼狀贅肉生長已至遮蓋瞳孔,且施行翼狀贅肉切除手術後,在角膜中心仍殘餘白斑疤痕,影響視軸的患者,分別為其施行自體半層角膜旋轉移植手術。手術技巧與一般半層角膜移植手術略同,手術進行順利,無手術中及手術後之併發症發生,手術後的角膜片一直保特清晰,病人視力有明顯改善,且無翼狀贅復發。

Ipsilateral lamellar rotating autokeratoplasty is indicated when a superficial nonprogressive central scar is present in an otherwise clear and normal cornea. By rotating an eccentric lamellar corneal button, the scar can be displaced peripherally and the clear cornea brought into the visual axis. Two cases of large pterygium with central cornea involvement were treated. After excision of the pterygium, residual scarring of the central cornea persisted, and ipsilateral lamellar rotating autokeratoplasty operations were performed in March and July 1985. The technique was comparable to routine lamellar keratoplasty. There were no intraoperative or postoperative complications. All grafts remained clear after 6 months and visual results were much improved. By rotation, ipsilateral lamellar rotating autokeratoplasty can provide normal Bowman's membrane into the area where Bowman's membrane has been destroyed by pterygium. No recurrence was noted in these two large pterygium cases.
