先天性白內障之屈光狀態

The Refractive Status of Congenital Cataract

本文報告58例接受白內障手術後,無其他眼部併發症或合併症之先天性白內障患者,40例兩側性、18例為單側性白內障。手術後平均26.2個月測量其角膜曲度半徑、屈光度、眼軸長度,並與正常眼比較,結果單側白內障之患眼眼軸長為23.67±1.95毫米,較另一健眼眼軸22.66±1.07毫米為長。全部男女白內障眼之平均眼軸長分別為24.59±1.54毫米及24.48±1.62毫米,均較同齡組之乎常眼男性23.36±0.71毫米,女性23.02±0.72毫米為長。角膜曲度半徑不論男女皆較同齡組為小。雙眼之白內障視力較佳眼,眼軸愈長視力則愈差,雙眼白內障視力較差眼及單側白內障之患眼則視力與眼軸長度並無關係。 本結果顯示先天性白內障之患眼其眼軸會變長,即其屈光狀態明顯的有近視傾向。

The corneal curvature, refractive error and axial length of eyes with congenital cataract were measured in order to investigate the effect of cataracts on the growth of the eyeglobe. A total of 58 cases (98 eyes), 40 bilateral and 18 unilateral, were studied. All these patients underwent lens extraction with mean follow up of 26.2 months without any complications relating to surgery or any associated ocular diseases. In the unilateral cataracts, the axial length of the cataractous eyes were significantly (P<0.01) longer than that of the fellow non-cataractous eyes, with the average axial length being 23.67±1.95mm and 22.66±1.07mm respectively. The average axial length of cataractous eyes, both in the male or female group, were significantly longer than that of the group with persons of similar age. However, the corneal radii of cataratous eyes were steeper than those of normal persons. From the results of this study we conclude that congenital cataracts appear to have a role similar to the vision blurring perturbations that trigger the development of myopia, axial myopia, in young animals. Early operation and full correction of the eyes are necessary in assuring the best possible vision.
