摘要


本篇利用前溯法探討台中榮總三年來(1982-1985),319例眼破裂傷資料。其中男性較多,男女之比為3:1。年齡分佈自8個月至72歲,以15歲以下孩童居首(37.3%),21-30歲次之(23.2%)。職業以工人最多(27.3%),學生次之(10.7%)。致傷原因以工作為主(34.2%),遊戲次之(26.0%)。致傷物以鐵片居首(25.1%),玻璃次之(12.5%)。眼內異物傷39例,其中28例成功地將異物取出。眼破裂傷依其傷害程度分五級,視力預後與傷害等級有關。治療最終視力在0.1以上者有48.7%,0.5以上者17.4%,28例(8.4%)導致眼球摘除。1例(0.3%)引起交感性眼炎。

A prospective study of 319 cases of penetrating eye injuries collected over a three-year period (Nov. 1982 to Oct. 1985) was made at Veterans General Hospital-Taichung. Males (239 cases) outnumbered females (80 cases) in a ratio of 3:1. Ages ranged from 8 months old to 72 years old. Children (under 15 Y/O) were the largest group (37.3%) followed by the 21-30 years old group (23.2%). Concerning the occupations, 27.3% were laborers, 10.7% students. As to the activity at the time of injury, paid work was the most frequent (34.2%), and play (26.0%) the second. Metal (25.1%) and glass fragments (12.5%) were the most common perforating objects. There were a total of 39 cases involving intraocular foreign bodies, which were removed successfully in 28 cases. The degree of injuries were divided into five grades. Laceration of the cornea or anterior sclera with or without iris prolapse was classified as grade 1. If lens damage was also present, then, grade 2. If accompanied by vitreous prolapse, grade 3. Perforating injuries with intraocular foreign bodies were classified grade 4. If both anterior and posterior segment involved, grade 5. The visual outcome varied with the degree of injury. The grade 1 injury had the best visual prognosis, while grade 5, the worst. As a whole, 48.7% of patients' final vision was better than 0.1 and 17.4% better than 0.5. Primary and secondary enucleation (or evisceration) were performed in 28 cases (8.4%). Sympathetic ophthalmia occured in 1 case (0.3%).
