摘要

本文報告7例外傷性前房出血性青光眼，以傳統之角膜鞏膜圓鋸術(Elliat's trephination)治療成功。加上51年報告2例並61年報告4例共13例，綜合討論之。該手術之效果良好，術後經過亦頗有一致性，即術後一週左右前房淨化不再出血，術後兩週左右出院。術後視力逐漸改良，經過一個月至10年之觀察，有9例(70%)恢復至有用之視力；但有2例(15%)於治癒後數年眼壓又增高，以慢性青光眼之型態出現。著者強調trepbination為一可行之手術療法。

The author reported 7 cases of secondary glaucoma due to traumatic hyphema which was treated with the traditional Ellot's trephination with successful results. Plus previously reported 2 cases on 1962 and 4 cases on 1972 makes the sum of 13 cases.
Four cases among them had been treated previously with paracentesis, washing out the blood from the wound of the corneal and by opening the anterior chamber and irrigation with little effects.
All cases at the time of operation were severe and suffering from symptoms of acute glaucoma with exhaustion. Vision limited to light sense or even worse. Trephination were effective in all cases and the post operative course were even and uniform. The anterior chamber usually cleared up in about one week and was able to discharge in about two weeks. Vision at the time of discharge were usually poor from light sense to count finger except in two cases. Vision improves gradually after discharge according the severness of the trauma damage.
Analysis and discussion on these 13 cases were made and revealed that 9 cases (70%) were improved to useful Vision from 6/15 to 6/5. But glaucoma developed in 2 cases (15%) on the later years as a type of chronic glaucoma. The observation period was from one month to ten years.
The author emphasize that the trephination is not only harmless and does not lead to further bleeding from the original wound but probably also a logical, safer and simple procedure in this troublesome condition.
