視網膜靜脈阻塞的電氣生理研究

Electrophysiological Studies of Retinal Vein Occlusion

摘要


以Visual Lab 5000 Series之網電圖(ERG)檢查儀器,研究正常眼與網膜靜脈阻塞眼之ERG變化。正常眼27隻,年齡由17至70歲,白光剌激強度為5焦耳。得亮適應及暗適應時A波潛伏期分別為13.1±1.1msec及14.6±2.6msec;振幅分別為41.6±11.5mV及198.4±88.6mV。B波潛伏期分別為30.4±1.6msec及40.5±5.7msec;振幅分別為135.6±38.9mV及258.4±41.lmV。藉頻率過濾器裝置,可得穩定之律動小波三個。其潛伏期分別為:16.9±1.1msec、23.0±1.4msec、27.3±2.4msec,總和之律動小波值為83.6±22.0mV。另以紅光及暗藍色光刺激,求得網電圖之正常圖形。 網膜靜脈阻塞病人31名,年齡由40至71歲,3例兩側性,研完結果發現:(A)患側眼與對側眼之律動小波值及暗適應藍光刺激之B波振幅,有統計學上有意義之差別。(B)以無灌流區大小、新生血管生成與否,有無玻璃體出血三項作疾病嚴重度之指標,則患側眼與對側眼之律動小波值之比值小於0.6或和暗適應藍光刺激之B波振幅比值小於0.6時,疾病嚴重度明顯上升。(C)ERG之變化可依疾病嚴重度歸納為四類。

The electroretinogram (ERG) of normal eyes and eyes with retinal vein occlusions (R.V.O.) were studied with Visual Lab 5000 Series ERG stimulating and recording system. 27 normal eyes in 21 persons with age between 17 and 70 were tested with white flash light of 5 jouls' intensity. The latencies of A wave under light and dark adaptation were 13.1±1.1 msec and 14.6±2.6 msec respecrively; the amplitudes were 41.6±11.5 mv and 198.4±88.6 mv. The latencies of B wave under these conditions were 30.4±1.6 msec and 40.5±5.7 msec; the amplitudes were 135.6±38.9 my and 258.4±41.1 mv. With the use of frequency filtration divice, 3 stable wavelets of oscillatory potential (OP.) could be obtained. The latencies were 16.9±1.1 msec, 23.0±1.4 msec, 27.3±2.4 msec individually. The O.P. value, defined as the sum of the amplitudes of each wavelets, was 83.6± 22.0 mv. Red flash light and dim blue flash light were also used to elicit the ERG responses. The results were recorded and presented. 31 cases of R.V.O., aged between 40 and 71 were tested. 3 cases were bilaterally affected. The results showed: (A) The difference of the scotopic blue B wave amplitude, O.P. value between the lesion eyes and the fellow eyes were statistically significant. (B) Using the non-perfusion area, neovascularization formation and vitreous hemorrhages as the severity index, the O.P. values ratio and/or scotopic blue B wave amplitudes ratio between the lesion eyes and the fellow eyes less than 0.6 were good indicators of the disease severity. (C) ERG pattern of all lesion eyes could be subdevided into 4 groups depending upon the waveform changes.
