眼窩動靜脈畸形以動脈栓塞術治療－病例報告

摘要

本文報告一位47歲男性，主訴左眼日漸外突，眼皮腫漲、靜脈曲張、眼壓上升、視力惡化已有四個月之久。頸動脈血管攝影術顯示出有眼窩動靜脈畸形。在上眼靜脈與內上顎動脈的眼窩下分支之間有不正常的瘻管。經放射線醫師用動脈栓塞術將該充盈血管阻塞後患者症狀獲得戲劇性地改善，視力又回復到1.0。對於這種動靜脈畸形的病例，欲以外科手術切除有實行上的困難與危險性。動脈栓塞術可以成功地治療部份病例，而且也可以使往後的外科手術變得較容易。

A 47-years-old male patient experienced progressive proptosis, lid swelling, periocular venous dilation, IOP elevation, and visual deterioration (os) for four months. Orbital arteriovenous malformation was confirmed by carotid angiography. Arteriovenous shunt was found between superior ophthalmic vein and infraorbital branch of internal maxillary artery. Selective arterial embolization with Ivalon was done by radiologist. The clinical symptoms and signs improved dramatically and visual acuiety returned to 1.0.
Initial surgical removal may be difficult, and embolization of feeding artery can decrease the size and vascularity of the malformation. It makes the subsequent surgical removal become more practicable and easier.
