小腦腫瘤引致上視性眼球向上震顫之一例報告
Up-Ward Beat Nystagmus Due to Cerebellar Tumor
摘要
小腦病變可引起多樣的垂直性異常眼球運動，其發生機轉與病灶之關聯性常受到注目。本篇所提出的上視性眼球向上震顫的病例是由肺癌轉移到小腦而引起的，此次討論病灶與眼球運動之關聯性。一位只有在月經期偶而成到頭痛的49歲女性，於74年12月25日突然感到眩暈。眼球運動檢查顯示此病例於注視正前方時不出現明顯之眼球震顫。而向上方注視時出現上視性眼球向上震顫。腦部電腦斷層檢查及開腦手術發現小腦有腫瘤。病理報告為轉移性黏液性腺癌。

Cerebellar tumors may cause a variety of vertial eye movement of pathological nature. Connections between the mechanism of this disorder and the site of lesion have received much attention. In this paper, the up ward beat nystagmus caused by cerebellar tumor transferring from lung cancer was discussed. The relation between the site of lesion and eye movement was discussed, too. A 49-year-old woman was admitted for a sudden onset of vertigo on Dec. 25, 1985. She suffered from occasional headache during the menstruation period. No other symptoms were notice EOG examination showed absence of nystagmus when looking straight forward and presence of it on gazing upward. CT scan and surgical operation revealed cerebellar tumor. Pathological study disclosed metastatic mucoid adenocarcinoma.
