釹雅克雷射穿鞏膜睫狀體光熱凝術

Nd:YAG Laser Thermal Transscleral Cyclophotocoagulation

摘要


應用釹雅克雷射熱凝性能與偏焦深度穿透之功能施行經由鞏膜之睫狀體熱觀術來治療頑強性青光眼。對於手術後無晶體性之患眼效果較佳,平均眼壓可由34.66mmHg降至15.83mmHg,然而對於絕對性青光眼之患眼,水晶體仍存在者效果較不明顯,平均眼壓由38 mmHg降至25 mmHg。然而有些病例之療效僅維持1~2個月,即再回至術前之眼壓。平均成功率僅達2/3,與環睫狀體冷凍術之成功率相差不大,但副作用卻遠比環冷凍術少。

The free running mode of Nd:YAG laser is the so called thermal mode. So far LASAG microruptor-Ⅱ is the only ophthalmic YAG laser which equipped with this function. Its special defocus and deep penetration properties let us can do the transscleral ciliary body destruction on intractable glaucoma eye instead of the traditional cyclocryotherapy. We follow up 60 eyes received YAG laser transscleral cyclophoto-coagulation. These include: neovascular glaucoma, aphakic glaucoma, absolute glaucoma. Intaocular pressure could drop from 45mmHg to 25mmHg after four to six months follow up. But within couple months some of them will returned to its original level in spite of maximal medications. The better results were seen on aphakic eye than phakic eye. Postlaser reaction is less than cyclocryotherapy. None of them developed bulbar atrophy.
