橋腦出血引起兩側水平注視中樞受損之病例報告
Bilateral Lesions of the Paramedian Pontine Reticular Formation (PPRF) in a Case of Pontine Hemorrhage
摘要
本文報告一例橋腦出血引起兩側水平注視中樞受損之病例，臨床表徵除了眼球水平注視運動完全喪失外，並表現出多樣化的神經症狀，包括右側第5, 7, 9, 10, 12腦神經麻痺，右側Horner症候群，左側肢體偏癱及小腦症狀。侵犯兩側水平注視中樞（即兩側中線旁橋腦網狀結構）的病灶，常合併多樣化的腦幹症狀，本例即為典型之例子。病人右眼角膜因面神經麻痺所造成之暴露性角膜炎經施以永久性眼臉縫合手術後明顯改善，神經症狀則繼續於復健科治療。

A case of pontine hemorrhage resulting in bilateral lesions of paramedian pontine reticular formation (PPRF) was reported. In addition to complete horizontal gaze palsy, the clinical manifestations included right Ⅴ, Ⅶ, Ⅸ, Ⅹ, Ⅻ cranial nerves palsies, right side Homer syndrome, left hemiplegia and cerebellar signs. Bilateral lesions of the PPRF is always associated with other brainstem findings, the case we reported here is a typical one. The right side exposure keratitis which was due to right side facial palsy was treated with permanent tarsorrhaphy with good result.
