瞳孔周期時間的臨床應用

Clinical Applications of Pupil Cycle Time

摘要


我們檢查239位眼科病人,包括屈調異常、白內障、弱視、網膜病變、青光眼、視神經病變…等,我們測其瞳孔周期時間,以探討瞳孔周期時間在臨床上應用不只限於視神經病變,只要片滕狀體以前視神經傳導途徑病變,都可以產生影響。但單純水晶體混濁,並不影響;而年齡卻是很重要影響因素之一。一般而言,弱視病人瞳孔周期時間應為正常,除非其本身非中心固視,由於固視方向偏差導致異常;對於深度,或不明原因,或困難治療之弱視患者,必須考慮是否有視神經病變,則瞳孔周期時間為重要而必備的檢查。對於無法合作或虹彩不正常的病人則瞳孔周期時間不易測量,Swinging light tes亦為簡單可靠的方法。

A thin horigental slit-lamp beam illuminating the pupil margin produces clearly visible pupil oscillations. These oscillations can be timed with a stop-watch, thus producing a measurement of the edge-light pupil cycle time. We measured pupil cycle time in 100 cases of refractive errors, 48 cases of amblyopia, 24 cases of cataract, 40 cases of retinal disease and 27 cases of other ophthalmic disease. We found that optic nerve lesion, optic tract lesion, extensive retinal disease, advanced glaucoma, large area of retinal detachment, deep amblyopia with eccentric fixation prolonged pupil cycle time. The comparison to Swinginglight test was disucssed.
