兩眼上斜肌麻痺病人之臨床觀察
Clinical Observations on Patients with Bilateral Superior Oblique Palsy
摘要
兩眼上斜肌麻痺在臨床上並非十分罕見，在診斷上亦非容易。我們報告六例兩眼上斜肌麻痺的病例，全部因為車禍造成並且都有意識喪失發生。所有的病例均可發現：(1)向左看時右眼上斜視、向右看時左眼上斜視。(2)兩側的Bielschowsky's test 均為positive。(3)大角度V-pattern的斜視。(4)大角度眼球外轉。(5) Primary position 時小角度或無上斜視。在經過Inferior oblique myectomy或Superior oblique tucking 或合併兩者手術後，所有的病例在眼球位置及臨床症狀上均有改善。

Bilateral superior oblique palsy is not an uncommon clinical finding. Yet, diagnosis of this disease remains somewhat difficult to the ophthalmologist. We present 6 cases of bilateral superior oblique palsy, of which traffic accident with initial loss of consciousness were all existed. All cases showed (1) right hypertropia when left gaze, left hypertropia when right gaze. (2) bilateral positive Bielschowsky's test. (3) large v-pattern esotropia (4) large degree excyclotorsion (5) primary position small or no hypertropia. Inferior oblique myectomy or superior oblique tucking or combined procedures were done. Postoperatively, the eye position and clinical symptoms were improved on all cases.
