橫竇硬腦膜動靜脈畸型引起視乳頭水腫－病例報告

Papilledema Due to Transverse Sinus Dural Arteriovenous Malformation-A Case Report

摘要

一例54例男性病患，主訴一年來有間歇性，短暫性的視力模糊；眼底發現兩側視乳頭水腫，腦部電腦斷層發現腦室正常，無腫瘤發現；Duplex超音波發現右側外頸動脈血流異常增加；脊髓穿刺發現腦脊髓壓偏高，腦脊髓液檢查正常；頸動脈血管攝影顯示右側大腦橫竇有硬腦膜動靜脈畸型，餵流血管為枕動脈，咽升動脈，及中腦膜動脈後枝；在施以餵流血管栓塞術後一週，右側外頸動脈異常血流及視覺障礙並無明顯改善。

We report a 54 years old male patient who has been bothered by progressive visual loss with frequent transient obscuration of vision for one year. Bilateral papilledema with IICP were noted and head CT was unremarkable. Carotid Duplex sonography showed abnormal increased flow in right external carotid artery. Carotid angiography revealved an arterio-venous malformation on right transverse sinus and fed by occipital artery, ascending pharyngeal artery and posterior branch of middle meningeal artery. Selective therapeutic embolization of three feeding arteries was performed yet with no improvement in vision and hemodynamics change of right carotid circulation.
