摘要

在白血病眼部侵犯中，視神經病變並不常見。然而視神經病變卻可以是白血病復發的唯一表徴。我們報告一例成人型急性淋巴性白血病復發以神經病變為初始表徵，經眼窩部放射治療後視力改善，但殘餘之癌細胞浸潤最終造成視神經萎縮。白血病患者即使在緩解期，例行性眼底檢查仍是相當重要，一旦發現視神經病變，立即以放射治療合併脊髓腔內及全身性化學治療可獲致明顯的效果。

Optic neuropathy is uncommon in ocular or orbital involvement of leukemia. However, optic neuropathy could be the only manifestation of relapse of leukemia. We report a case of initial relapse of adult acute lymphoblastic leukemia in optic nerves. Vision got improved after bilateral orbit radiation, but residual cancer cell infiltration lead to optic atrophy finally. It is important to perform routine ophthalmoscopic examination in leukemia, even during the remission state. In case of leukemia optic neuropathy, immediate radiotherapy combined with intrathecal and systemic chemotherapy may result in remarkable effects.
