纖維素膜－一種白內障術後的早期併發症
Fibrinous Membrane-An Early Complication after Cataract Surgery
摘要
本文報告三例白內障術後發生纖維素膜形成的病例。三病例於白內障術後皆出現纖維素膜覆蓋於瞳孔區域，而此時前房則僅表現輕度的發炎反應，故可和感染性眼內炎做鑑別診斷。二病例的纖維素膜出現於術後早期（分別在術後第7天及第8天發生），另一病例則在術後較晚期發生（術後10天以後才發生）。三病例的纖維素膜經約2星期的類固醇及睫狀肌麻痺劑治療後完全消失，然而有一病例最後發生瞳孔阻斷性青光眼而需以鐳射虹膜造孔術治療。本症之成因目前仍不確定，我們建議對具有危險因子的病人，在白內障手術時應同時施行手術虹膜切除術，以避免術後瞳孔阻斷性青光眼的發生。

We report three cases of fibrinous membrane formation after cataract surgery. The membrane developed over the pupillary area with only mild inflammatory reaction in the anterior chamber. Two cases developed fibrinous membrane respectively on the seventh and eighth days after surgery. The fibrinous membrane of the third case occurred ten days later postoperatively. The membrane of all cases dissolved after about two weeks of treatment with steroids and cycloplegics. However, one case developed pupillary block glaucoma later and required subsequent laser iridotomy. Possible causes and risk factors of fibrinous membrane formation after cataract surgery will be discussed. We recommend to perform iridotomy during cataract surgery in patients with risk factors to prevent future pupillary block glaucoma.
