蠟桿菌引起之外傷後猛爆性眼內炎－兩例病例報告
Posttraumatic Fulminant Endophthalmitis Caused by Bacillus Cereus-Two Cases Report
摘要
本篇報告兩例因蠟桿菌所引起之外傷後猛暴性眼內炎的病例。他們兩位都有眼內異物及猛暴性眼內炎合併有角膜周邊浸潤。他們兩位在我們努力治療之下，仍難逃眼內容物摘除術的命運。蠟桿菌與枯草桿菌在外形極為相似，但是生化及臨床表現則相當不同。在桿菌種中，蠟桿菌的破壞性最強，相對地預後也最差。對一個外傷後猛暴性眼內炎且合併有眼內異物者，我們必須強烈懷疑是蠟桿菌所引起的眼內炎。對於蠟桿菌眼內炎的臨床表徵，預後及治療，本篇將一一加以介紹及探討。

Two cases of posttraumatic fulminant endophthalmitis caused by Bacillus cereus were reported. Both of them had intraocular foreign body and fulminant endophthalmitis with corneal peripheral ring infiltration. Both of them inevitably received evisceration. Bacillus subtilis and Bacillus cereus are similar in morphology but they are quite different in lecithinase test and clinic course. Because Bacillus cereus is the most necrotizing in Bacillus species and offers the worst prognosis, in posttraumatic fulminant endophthalmitis with intraocular foreign body, it must be highly suspected and clindamycin is the drug of choice.
