眼瞼穿刺傷後之上斜肌麻痺－病例報告
Superior Oblique Muscle Palsy Following Penetrating LID Injury-Case Report
摘要
本文報告一眼瞼穿刺傷後造成上斜肌麻痺之病例。病人於傷後一個月主訴有垂直性複視，且外觀上有頭位偏斜的現象。臨床檢查發現左眼上斜視，外旋，即下斜肌過度作用。經下斜肌切除手術(IO myectomy)後，臨床症狀大為改善。眼瞼穿刺傷後造成上斜視的情形較為罕見，因此提出報告。

Superior oblique muscle palsy following eyelid penetrating trauma is rare. The more common response to trauma is hypotropia or acquired Brown's syndrome. We reported a 11-year-old girl who noticed diplopia following a penetrating trauma to the superior nasal quadrant of left eyelid. The clinical features included loss of depression, hypertropia in primary position, and torsional imbalance. Park's three-step test confirmed decreased superior oblique muscle function. The results of clinical examinations and the mechanisms were discussed.
