不對稱水平斜視合併具髓鞘視網膜神經纖維－病例報告

摘要

本篇報導一六歲女童，其左眼具高度軸性近視，弱視，眼底呈廣泛性之具髓鞘視網膜神經纖維等三項特徵，右眼呈中高度遠視且合併不尋常之斜視表現。其眼位變化於交替遮蓋時，呈同方向不等量之水平移動，即左眼固視物體時右眼呈輕微外上斜視，及右眼固視物體時左眼呈內斜視，經予以配鏡矯治其左側弱視眼，而右眼予以平光鏡片，以期同時達到部分遮蓋之效果，眼位則呈輕微右眼外上斜，其最終視力及外表約有令人滿意之結果。過去雖國內外陸續有學者提出具上述特徵之病例報告，但因其罕見之眼位變化，故除詳述臨床表徵及吾人之治療及成效外，並嘗試尋求合理解釋並討論之。

We describe a 6yio girl who presented asymmetric horizontal stabismus associated with myelinated nerve fibers, high axial myopia and refractory amblyopia, which was recognized as a syndrome since 1914. But the unusual eye position change in our case that against Hering's law was never mentioned before. After prescribing glasses, her vision improved and eye position became satisfied after following up for 1.5 years. We reviewed literatures to seek simulate mechanisms and reported the possible explanation of our case. Though the exact cause is not clear yet, we hope there will be more cases to be discussed in the near future, so we can realize this interesting eye position change more clearly.
