眼科超音波篩檢在眼科疾病診治之應用

Echographic Screening in the Diagnosis and Management of Ophthalmic Diseases

摘要


為研究超音波在眼科門診第一線篩檢之價值,我們回溯1990年1月至1994年6月於眼科門診初診作超音波檢查之病例共1607例。分析統計作超音波之原因,以介質混濁最多,佔59.9%,檢查後診斷不變者佔71.5%,診斷改變者,佔14.9%,檢查有助於確切診斷者佔13.6%,有37%超音波檢查比臨床檢查發現更多病變。在治療方面,手術計劃不因為超音波而改變者佔65.5%。因超音波檢查而改變手術計劃者佔22.3%,檢查結果幫助了治療方式計劃者佔12.2%,向以上統計可見,在配合其他臨床資料下,眼科超音波之第一線篩檢可幫助眼科醫師迅速判斷病因,偵測病變範圍及提拱治療方式之選擇。

For evaluating the value of ophthalmic echographic screening in ophthalmic out patient clinic, 1607 cases underwent ultrasound examination on their first visit to ophthalmic clinic from January 1990 to June 1994 in National Taiwan University Hospital were reviewed. Medium Opacity was the most common reason (59.9%) for ultrasound examinations. After the examination, 71.5% had no change of pre-examination diagnosis, 14.9% had change of diagnosis. The examination helped differential diagnosis of the lesion in 13.6% of the cases, 37.0% of cases had more pathologies found than clinical examinations. For those underwent surgical treatment, 65.5% had no change of treatment plan after the examination, 22.3% had change of treatment plan after the examination, and in 12.2% of the cases, the echography assisted in planning the treatment. These results showed that echographic screening in ophthalmic clinic was valuable in the diagnosis and treatment of various ocular disorders.
