青光眼患者於非接觸式二極體雷射經鞏膜睫狀體光凝固術治療後產生之黃斑部囊狀水腫－病例報告

Cystoid Macular Edema Following Noncontact Diode Laser Transscleral Cyclophotocoagulation in the Glaucoma Patient-A Case Report

摘要

本篇報告一例青光眼病患經非接觸式二極體雷射經鞏膜睫狀體光凝固術治療後併發黃斑部囊狀水腫的病例。病人為31歲女性，左眼原發隅角開放性青光眼經藥物與雷射小樑整型術治療後效果不顯著，故改用非接觸式二極體雷射光凝固術治療。術後眼壓控制良好，卻於一個月後併發黃斑部囊狀水腫致左眼視力模糊。給予局部類固醇，局部與口服非類固醇抗發炎藥物治療後視力漸回復至1.0。因非接觸式二極體雷射經鞏膜睫狀體光凝固術引發之黃斑部囊狀水腫十分罕見，特提出報告。

We report a case of a 31 year-old women with primary open-angle glaucoma who developed cystoid macular edema 1 month following noncontact diode laser transscleral cyclophotocoagulation.
The patient presented poor control of intraocular pressure after maximal tolerated medical therapy and Argon laser trabeculoplasty before she received this procedure. After diode laser transscleral cyclophotocoagulation, the intraocular pressure was well controlled but the visual acuity decreased to 6/30 due to cystoid macular edema. Intensive treatment with oral indomethacin, topical steroid and topical indomethacin were given, and the visual acuity returned to 6/6 as previous level.
From this case, we demostrated that cystoid macular edema is one of the potential complications of the diode laser and should be managed cautiously.
