本文提出由非接觸式眼壓計(pulse-air non-contact tonometry)所造成角膜移植術後(penetrating keratoplasty)眼前房內灌氣的兩例病例報告。分析非接觸式眼壓計的機器和此二事件的關係。兩例的術後過程皆十分平順,直到術後第三及第四天,以非接觸式眼壓計測量眼壓後,意外地發現前房內灌氣。儘管非接觸式眼壓計對於測量眼壓十分便利,但是可能使術後的角膜傷口產生前房灌氣。因此,對於角膜移植術後或有角膜傷口的病人,使用非接觸式眼壓計測量眼壓時,應特別注意潛在併發症的產生。
We report two cases of air intrusion into anterior chamber after penetrating keratoplasty by pulse-air non-contact tonometry (NCT). The relationship between mechanics of pulase-air NCT and these events is analyzed. The post-operative courses of these two patients were uneventful. But 3 or 4 days after the penetrating keratoplasty, some air was accidentally introduced into the anterior chamber while measuring intraocular pressure by the pulse-air NCT. The corneal wound was either in single running sutured or double run-fling sutured. The NCT machine was in good condition and properly calibrated. It is convenient to measure intraocular pressure by the pulse-air NCT. But, the accidental forced entrance of air into the anterior chamber through postoperative cornea wound by this machine is possible. This potential complication should be kept in mind while using pulse-air NCT in cases receiving keratoplasty or other surgery with large corneal wound.
