青光眼網膜神經纖維層的臨床評估(Ⅵ)視神經盤出血之特性與網膜視神經纖維層及視野缺損之關係

本文收集馬偕醫院青光眼特別門診中25位發生視神經盤出血的病患,觀察分析其特性,及與視網膜神經纖維層、視野變化之關係。視神經盤出血於青光眼病人中發生率以正常眼壓性青光眼最高。出血多出現於視神經盤下顳側,持續數週後會自然吸收,有一半機會會再發生出血。出血的部位常可見網膜神經纖維層缺損(以楔狀缺損為主),且在相對應位置的視野出現變化。我們推測青光眼的病理機轉可能以血管血流異常為主,而是否因出血直接造成青光眼性的傷害,還沒有明確的證據支持。

Twenty-five cases of disc hemorrhage were collected from the glaucoma clinic to evaluate its characteristics and correlations with changes in retinal nerve fiber layer and visual field. Disc hemorrhage occurred most frequently at inferiotemporal quadrant (60%). Its recurrent rate was 44%. The prevalence rate of disc hemorrhage was highest in the normal tension glaucoma group (15%), followed by the primary open angle glaucoma group (2.6%). In eyes with disc hemorrhage, there was no difference in introcular pressure between visits with and without bleeding. Disc hemorrhage was indeed associated with nerve fiber layer defect and visual field changes.
