視野缺損對青光眼病患視覺功能之影響
The Influence of Visual Field Defect on Visual Function in Glaucoma Patients

青光眼患者常會有各種型式之視野缺損存在，但視野缺損對視覺功能之影響，目前仍不是非常清楚。本研究之主要目的，乃針對重度青光眼患者，以VF-14視覺功能評估表，評估其視野缺損對視覺功能之影響。全部53位患者，大部分都仍可維持很好之視覺功能，視覺功能得分低於80分者只有六位，而視覺功能得分之高低，與兩眼視野缺損之總合有明顯關連，視野缺損愈大，視覺功能得分便愈低。53位患者中，雙側患者佔25例，單側28例；雙側青光眼患者之視覺功能，稍微低於單側青光眼患者，但二者尚無明顯差異存在。視野缺損之形式對視覺功能之影響，與雙眼視野缺損之嚴重度有關，單眼出現中心或顳側島狀視野時，視覺功能不一定會受影響，但如雙眼皆只剩中心島狀視野，視覺功能將明顯降低。

Glaucoma causes several types of visual field defect, but the correlation between the severity of visual field defect and visual function is not well known. The purpose of our study is to determine the influence of visual field defect on visual function in patients with advanced glaucoma, according to the VF-l4 visual function test. There were 53 cases with advanced glaucoma. Most of them could maintain good visual function, only six cases had less than 80 scores of visual function test. However, the correlation between visual field and visual function was statistically significant among patients with advanced glaucoma. In all cases, twenty five cases were bilateral, twenty eight cases were unilateral. The visual function of bilateral cases was worse than unilateral cases, but the difference of visual function between them was not statistically significant. With different types of visual field defect, the effect on visual function correlated with the severity of bilateral visual field defect. The visual function iii unilateral cases with central island or temporal island visual field may not be affected, but bilateral cases with central island visual field seemed to have poor visual function.
