Anatomical Landmark for Removing a Double-Perforating Metallic Foreign Body Which Incarcerated at the Very Posterior Aspect of the Eyeball-A Case Report

摘要


病例為一位30歲男性,因工作時被反彈金屬片射入左眼,造成角膜穿孔及外傷性白內障。急診當天便進行角膜裂傷修補,摘除外傷性白內障,但金屬異物穿透眼球且卡於眼球正後方。我們利用解剖學上視神經、上斜肌之後側附著點、vortex靜脈的相關位置定位,從眼外取出異物。

A 30 years old male had injury to his left eye by a rebounding piece of iron during work. Corneal perforation with traumatic cataract were noted, skull routine and emergent orbital CT disclosed a metallic intra-ocular or intrascleral foreign body. Two consecutive operations were performed to repair the injury and to remove the foreign body. Unfortunately, the foreign body could not be found intraocularly. Secondary orbital CT revealed the foreign body may have double-perforated the eyeball and rested just at the very posterior pole of the eyeball. The third operation was done again, intending to remove the foreign body. Using the optic nerve, posterior insertion site of superior oblique muscle and vortex vein as anatomical landmark, the foreign body was removed successfully. We present this clinically interesting case and review the anatomy of the posterior aspect of the eyeball.
