單側柯氏症合併對側先天性視網膜血管巨大症－病例報告
Unilateral Coats' Disease with Contralateral Congenital Retinal Macrovessel-A Case Report
摘要
目的：本篇報告壹單側柯氏症合併對側先天性視網膜血管巨大症之病例。 方法：病例報告。 結果：病人為35歲女性，左眼因無意間遮眼而發現視力模糊，經檢查為柯氏症，右眼在眼底檢查時，發現一巨大血管通過中央黃斑部。雙眼螢光眼底攝影發現左眼有明顯的柯氏症毛細血管擴張與染料滲漏，右眼無染料滲漏現象。左眼經多次局部雷射光凝固治療後，最佳矯正視力可達零點陸。右眼未做任何處置，病人無抱怨任何不適且視力可矯正至壹點零。 結論：柯氏症合併對側之先天性視網膜血管巨大症十分罕見，故提出報告並討論其可能原因。

Unilateral Coats' disease combined with contralateral congenital retinal macrovessel is rare. We reported a 35-year-old female complaining of poor vision in her left eye. The best visual acuity was 1.0 in the right eye and 0.4 in the left eye. Fundus examination revealed multiple microaneurysm, hard exudate, and macular edema in the left eye; an abnormal vascular segment across the macular region was found in the right eye. Fluorescein angiography showed leaking telangiectasia, macroaneurysm in the left eye but no abnormal leakage in the right eye. We present the clinical and fluorescein angiographic features of a case of unilateral Coats' disease with contralateral congenital retinal macrovessel. We porposed that the pathogenesis of these two conditions was abnormal embryogenesis of endothelium cells of retinal vessels.
