Retinoblastoma Presented Unusually as Orbital Cellulitis, Spontaneous Hyphema, and Dislocated Lens-A Case Report
以自發性前房出血，水晶體脫位，及眼窩峰窩組織炎為臨床表現之視網膜胚胎母細胞瘤－病例報告
摘要
本病例報告一位四歲兩個月大的小男孩，在輕微頭部外傷後產生左眼嚴重眼窩水腫，類似蜂窩組織炎，並合併前房出血。就醫後之電腦斷層及磁振造影發現其左眼有一具鈣化點的眼內腫瘤，同時併有水晶體脫位的現象。左眼眼球摘除後之病理報告顯示為廣泛壞死的視網膜胚胎母細胞瘤。視網膜胚類胎母細胞瘤合併水晶體脫位在文獻上極少被報告，而其所造成類似蜂窩組織炎之表現亦非常少見，常常會讓臨床醫師誤以為腫瘤已擴散到眼窩。事實上這通常是腫瘤細胞大量壞死所引起的免疫反應，並不意味著腫瘤之眼球外擴散。

A 4-year-old boy demonstrated as severe orbital cellulitis and total hyphema of his left eye after a minor head injury. An intra-ocular tumor with calcification and lens dislocation were demonstrated by orbital CT and MRI. Retinoblastoma with massive tumor necrosis was proved by pathology after enucleation. Retinoblastoma with subluxated lens had rarely been reported in the literature. Orbital swelling mimicking orbital cellulitis is also an uncommon presentation of retinoblastoma, and it does not necessarily indicate extrascleral extension of tumor.
