淚腺復發性木村氏症―病例報告

摘要

21歲男性，入院時兩眼上眼皮浮腫且左眼皮下有一顆腫瘤，病人曾經在三年前接受左上眼瞼腫瘤切除手術，而且診斷為木村氏症。不幸腫瘤在兩年前開始復發，而且越長越大。眼部電腦斷層掃瞄發現左眼瞼下有一顆腫瘤，且兩邊眼睛的眼外肌出現脹大的情形，病人接受腫瘤切除術合併眼瞼成型術，病理報告呈現典型的木村氏症。木村氏症通常發生在十幾歲的年青人而眼部侵犯是很少見。復發性的木村氏症很少發生在手術切除後。這位病患在接受手術治療後效果很好。

A 21- year- old young man presented with bilateral eyelids swelling and had a palpable mass under the skin of left upper eyelid. The patient had received excision of a tumor mass over the left upper eyelid three years ago under the diagnosis of Kimura disease. Unfortunately, the mass lesion had recurred and been gradually growing in past two years. The orbital CT showed a tumor mass beneath the skin of left eyelid and enlarged bilateral extraocular muscles. Laboratory studies revealed peripheral eosinophilia with elevated IgE level. We completely excised the tumor mass in combination with blephoplasty and got good cosmetic results. Pathology reported that multiple germinal centers were infiltrated with massive lymphocytes and eosinophils had diffusely infiltrated the whole tumor lesion, which characterizes Kimura disease. Typical Kimura disease develops predominantly in teenage patients and an orbital lesion is rare. Recurrence of Kimura disease scarcely happened after complete surgical excision. In this report, we confirmed in the case of the 21year-old young man with recurrent orbital Kimura disease. After surgical excision with simultaneous blepharoplasty, a good surgical outcome and satisfactory cosmetic results were achieved.
