摘要

相關流行病學及臨床研究均顯示糖尿病視網膜病變是第2型糖尿病患者最為常見的併發症之一。在台灣地區，過去的社區性研究發現糖尿病視網膜病變的盛行率在15-45%，而年發生率則為4.8%，就流行病學及預防醫學的觀點而言，如何透過有效的篩檢策略達到視網膜病變早期偵測及早期治療是重要的課題。此外，為了解糖尿病視網膜病變的疾病率，我們必須考慮其疾病自然史以進一步預防視網膜病變的發生。同時也要評估相關的危險因子以做到個人預防的目標。本文即是以流行病學及經濟評估的角度探討第2型糖尿病患者其視網膜病變相關議題，希望藉由系統性的文獻探討，以提供更多公共衛生訊息，並作為衛生機關日後建立相關篩檢及保健政策的參考，以便做好糖尿病病人之健康管理工作。

Epidemiologic and clinical studies have shown that diabetic retinopathy is the most common and serious complication among type 2 diabetics. In Taiwan, the previous community-based study showed that the prevalence and annual incidence of diabetic retinopathy estimated are 15% to 45% and 4.8%, respectively. From the viewpoints of epidemiology and preventive medicine, the most important topic of preventing diabetic retinopathy is how to achieve early detection and early treatment, through effective screening tools. Besides, to understand the morbidity rate of diabetic retinopathy, we must take into consideration the progression of natural history of diabetic retinopathy in order to be ahead of preventing occurrence of diabetic retinopathy. In order to reach early prevention of diabetic retinopathy for individual patients, one must probe through the relevant risk factors the occurrence of diabetic retinopathy and its transitional influence, to achieve personal prevention. This article is discussed on the epidemiology and economic evaluation of diabetic retinopathy among type 2 diabetics. We hope that this literatures review could provide more information to establish screening strategies and health management programs for the popular diabetic subjects.
