摘要

目的：報告一個智障兒童的兩側性視網膜巨大裂孔併視網膜剝離以及視神經萎縮的經驗
方法：病例報告
結果：一位Rubinstein-Taybi症候群的14歲智障男童，因視力不良至本院眼科求診。眼底檢查發現兩側視網膜剝離及視神經萎縮，頂起鞏膜時可見兩眼周邊基底部視網膜在鼻側下方都有撕裂現象(oradialysis)，視網膜撕裂範圍有四個鐘點，兩眼均達到巨大裂孔的程度。從病史及臨床上之觀察，判斷是該童受到眼部傷害而引起的後遺症。雖經經過鞏膜環扣手術治療，病童左眼網膜貼回，但是視力仍然不良。
結論：本文提供一個難得的眼科經驗，並且提醒家長在兒童眼部受傷以後早日就醫檢查治療的重要性。

Purpose: to report a case of bilateral rhegmatogeneous retinal detachment in child with Rubinstein-Taybi syndrome.
Method: case report
Result: A 14-year-old boy, a case of Rubinstein-Taybi syndrome, suffered from loss of vision for several months. Fundus examination showed bilateral retinal detachments with macular area involved. There was also optic atrophy noted. Intraoperative findings included oradialysis with giant tears in bilateral infereonasal quadrants. Although surgery was performed uneventful with retina reattached in the left eye, the visual prognosis was still poor.
Conclusion: This case report reminds the parents and ophthalmologists the importance of early ocular examination when a child is suffered from ocular trauma.
